The mosquito is the biggest human killer on the planet.Malaria alone kills 1.2 million people yearly yet this is not the only killer disease spread by mosquitoes spread .Yellow fever,Elephantiasis,Dengue Fever,Filaria and West Nile virus are other sometimes killer viruses spread by mosquitoes.There are also several other viruses though often mild,kill if they are the severe strains like the encanphalitis strain of viruses.

The second largest killer of humans is ...wait for it... humans!
I think the rat is the first animal that is responsible for the most human deaths.In my country itself a few years back hundreds of people died due to plague that could not be controlled for three months at a stretch.As the users above as stated the mosquito too goes no less than the rat in killing people.Anywhere they find accumulated water,they breed.It is difficult in my house to keep a bucket of water for bathing standing for few hours as I invariably find that mosquitoes have bred in it and the larva is visible too.It it so terribly nauseating.We get very hot water due to the hot summers here so we store the water in buckets for bathing later but this is what happens when the water is kept standing for long.So invariably the buckets have to be kept covered.My country is a mosquito-infested place and diseases like malaria, yellow fever and Chikungunya are household names here that rob so many people of their lives.So I would blame both these animals for being responsible for the most number of human deaths....
A lot of people will say mosquito or rats however I must insist these animals are innocent!
They may carry viruses, or fleas with bacteria, but it is the virus, or the bacteria that kills humans, not the mosquito or the rat, so please cross these animals off your list!
The animal that is directly responsible for the most human deaths is the Asian Viper - a snake, these kill thousands of people every year, most deaths are unreported, happening in remote villages, and are simply treated as a part of every day existence in those areas. An estimated 15,000 to 20,000 people are killed each year by Asian Vipers.
The Black Mamba would be second, and third is the Asian Elephant.
Add - this assumes you are not counting people, after all people kill more people than other animals kill people.
When we visited the Kruger National park in South Africa, I am sure that the ranger told us that the hippopotamus is responsible for the most human deaths. Hippopotamus have always been one of my favourite animals and we were warned not to go near them because they are not as docile as they appear.
Hippopotamus are very aggressive and unpredictable animals and will over turn boats without warning, killing people on board. They are especially aggressive if they have calves.
Do not be fooled into believing that hippopotamus are docile and slow, one of the things that makes them so dangerous is the fact that they are unafraid of humans.

I would say that it would be the common Rat or Flea through disease. They may not be directly responsible but think of all the people they have killed indirectly with diseases such as malaria.
Rats are responsible for Hanta virus, Rat bite fever, Murine typhus and many more. A good example would be the Bubonic plague which kills around two thirds of humans in close to four days. The great plague of London killed an estimated 100,000 in only one year and this was all due to a flea! These cretures live amongst us and sleep in our Attics underneath our garden sheds and in our walls. That seems pretty deadly to me.
I think the animal directly responsible for the most human deaths is other humans. The United States alone sees more than 15,000 homicides per year...and that's street crime. That doesn't even look at the incidence of death during wars or political actions or in homicide stats in every other country in the world....
I have heard that the common mosquito is responsible for more human deaths worldwide than any other living creature.
Think of all the disease vectors that mosquitoes carry.
